The numbers are just measurements: an 18650 is 18mm wide and 65mm long, a 21700 is 21mm by 70mm. Bigger cells usually mean longer battery life at the same power, but only if your mod has room for them.

Decoding the numbers
Cylindrical lithium-ion cells are named by their size. The first two digits give the diameter in millimetres, the next two the length in millimetres, and the final 0 tells you it is round. So an 18650 is 18mm across and 65mm long, and a 21700 is 21mm across and 70mm long.
All three sizes share the same chemistry family and voltages: 4.2V full, around 3.6 to 3.7V nominal. The difference is space. A fatter, longer tube holds more active material, which means more capacity and usually a higher safe current too.
You will also see 26650 cells (26mm by 65mm) in a few older or chunkier mods. They are rare in new devices but still sold as replacements.
Capacity vs current: the trade-off
Inside any one size, cell makers trade capacity against current. A high-capacity 18650 might hold around 3500mAh but only be rated for about 10A continuous, which is fine for a torch and poor for an 80W mod. A high-drain 18650 built for vaping typically sits between 2500 and 3000mAh with a much higher amp rating.
Moving up to a 21700 gives you more room on both fronts. Well-known vape-grade 21700s typically sit around 4000mAh while still handling the current a single-cell mod needs at higher wattage.

What fits your mod?
The battery door, the manual or the spec sheet will tell you exactly which sizes the mod accepts. Do not guess from what physically squeezes in, because contacts and spring pressure are designed around specific sizes.
Plenty of mods are single 18650, like the Smok X-Priv Solo. Some take either: the Lost Vape Centaurus N100 runs a single 18650 or a single 21700, using an adapter for the smaller cell. Others have a sealed internal battery and none of this applies.

What does your mod's manual say about batteries?
Buy genuine high-drain 18650s with an amp rating that covers your wattage. Nothing bigger will fit.
Run 21700s for the longest battery life. Use 18650s in the supplied adapter if you already own good ones.
There is nothing to buy. Charge it with a decent USB plug and replace the device when the battery fades.
Buy cells in identical pairs, same model and age, and keep each pair together for life.
Is switching to 21700 worth it?
If your mod takes both, a 21700 is the easy win for battery life. A typical 4000mAh 21700 against a 3000mAh 18650 is about a third more capacity, which is the difference between topping up mid-afternoon and getting through to the evening.
The catch is size. 21700 mods are a touch wider and heavier, and a 21700 weighs noticeably more than an 18650. For a pocket mod at modest wattage, a good 18650 is still perfectly sensible and far easier to find.
If you vape higher power on a single cell, the bigger cell's extra current headroom also helps it run cooler and sag less under load.
Sleeves, adapters and bodges
An 18650 rattling about in a 21700 bay can lose contact mid-puff or sit at an angle against the contacts. That is why mods that accept both include a purpose-made adapter sleeve. Use that one, or an identical replacement from the same maker.
Never pad a cell with paper, foil or tape to make it fit. Foil in particular is conductive and can bridge the positive and negative. Equally, never force a larger cell into a smaller bay: a tight fit can tear the wrap as it goes in, and a torn wrap near the top is a short waiting to happen.
Charge 21700s in a charger with bays long enough for 70mm cells. Most modern chargers handle both sizes.
